NOTE:  We strongly recommend the use of a computer device with the Chrome browser.  Estimated Time: 15-30 minutes You may save your application at any time during the process with your login credentials.

Once you enroll a new student, you will be required to provide the following information below this page. You may start your Online Enrollment without all of the documents, however, you will need to upload the documents to complete Online Enrollment.

The following items are needed in order to complete the enrollment process (please upload a copy of the required documents to the Document Upload section, which lies closer to the end of the application).

• Valid Email Address(required) Parents/Guardians will need to have a valid email address. Do not use a student's email address.

• Immunization Record (required document) All students entering school are required to have proof of up-to-date immunizations

• Proof of Address (required document) Documents provided must be in the parent/guardian’s name. Acceptable documents include a current utility bill (electricity, gas, water)

• Previous Enrollment (required document) Previous Transcripts/Report Cards, Previous Enrollment Date(s), Previous Enrollment School(s)

 

• Documentation of Age (required document) Birth certificate, passport, hospital certificate, etc.

• Custodial or restraining court orders(optional, if applicable) Please inform your student's site of any court orders or legal documents of your child's custody, if applicable
